OFFICE POLICIES

IN OFFICE AND TELE-HEALTH APPOINTMENTS

Initial appointments will be 60 minutes in length.
Follow-up appointments will be 15–30 minutes in length.

CANCELLATIONS

A notice of cancellation must be given 24 hours in advance of your appointment.
Failure to notify us of a cancellation may result in a cancellation fee.

PAYMENTS

Payments are due at the time of your appointment or visit.
Acceptable payment methods: insurance, credit/debit card, or cash.
Outstanding balances must be settled before they can be seen.

PRESCRIPTIONS

Please let your provider know of any refills needed at least one week prior to running out.
You may be asked to schedule an in-office appointment in order to evaluate and refill prescriptions.

MINORS

All patients under the age of 18 must be accompanied by a parent or legal guardian

RECORDING

Due to the sensitive nature of the services we provide, no video or audio recording is allowed.

ANIMALS

Only working service animals are allowed in our office.
